Losing Eligibility

Medical, dental, and vision coverage for you and your covered family members ends on the last day of the month in which you fail to meet eligibility requirements. Coverage for all other plans generally ends on the last day of employment.

In some cases, you can continue medical, dental, vision, and GuidanceResources EAP coverage, as well as your Health Care Flexible Spending Account contributions, through COBRA. A COBRA packet will be mailed to you within 14 days of your loss of coverage.
